일산화탄소 중독의 개요

  • 고 농도의 일산화탄소(CO)를 많이 호흡하면 일산화탄소 중독에 걸릴 수 있다. 
  • 짧은 시간 내 높은 농도의 일산화탄소를 호흡해 생긴 일산화탄소 중독을 급성 일산화탄소 중독이라 한다. 
  • 여러 날 동안 고 농도 일산화탄소를 호흡해서 생긴 일산화탄소 중독을 만성 일산화탄소 중독이라 한다.

  • 헤모글로빈은 정상적으로 적혈구 속에 들어 있다
  • 그 헤모글로빈은 호흡할 때 폐포 속으로 들어온 산소를 전신 모든 계통의 기관과 조직에 있는 세포들에게 날라다 주는 역할을 주로 한다.
  • 일산화탄소와 산소가 폐포 속이나 핏속에 동시 있을 때 산소가 헤모글로빈에 붙는 친화력보다 일산화탄소가 헤모글로빈에 붙는 친화력이 200배나 더 강하다.
  • 어떤 이유로 일산화탄소를 정상 이상으로 더 많이 호흡할 때는 헤모글로빈이 산소를 전신의 신체의 모든 계통의 모든 기관과 조직의 세포에 날라다 주는 대신 일산화탄소를 전신 모든 조직 세포에 더 많이 날라주게 된다. 이때 일산화탄소에 중독 될 수 있다.
  • 일산화탄소 중독의 중증도에 따라 전신의 모든 조직의 세포들에 산소결핍증이 생기고 심하면 결국 조직의 세포들이 질식 된 상태에 빠진다.
  • 다시 설명하면, 일산화탄소의 농도가 높은 공기를 호흡할 때는 적혈구 속에 있는 헤모글로빈에 산소가 부착되는 대신 일산화탄소가 헤모글로빈에 부착해서 일산화탄소 헤모글로빈(일산화탄소 혈색소/Carboxyhemoglobin/COHb)이 된다.
  • 그 때문에 헤모글로빈이 신체 각 조직 세포들에게 필요한 만큼 산소를 충분히 나를 수 없게 된다.
  • 이런 경우, 몸속에는 헤모글로빈의 양은 정상적으로 충분히 있으나 신체 내 산소결핍증이 생기게 되고 결국에는 일산화탄소에 중독된다.
  • 즉 혈중 헤모글로빈 농도가 정상이지만 심한 빈혈 상태의 증상 징후가 생길 수 있다.

일산화탄소 중독의 원인

  • 잘못 설치한 난방장치,
  • 불안전하게 설치한 환기장치,
  • 환기가 잘 되지 않는 밀폐된 장소에서 석유석탄또는 숯 등을 태울 때,
  • 밀폐된 자동차 내에서 오랫동안 자동차 엔진을 끄지 않은 상태로 있을 때,
  • 자동차 엔진을 오랫동안 끄지 않은 상태에서 주차시킨 밀폐된 차고 내에 오랫동안 있을 때,
  • 화재로 생긴 높은 농도의 일산화탄소를 호흡할 때,
  • 그 외로 일산화탄소의 농도가 높은 공기를 호흡할 때 일산화탄소 중독에 걸릴 수 있다.
  • 화재가 난 장소에서 화상은 입지 않아도 일산화탄소나 그 외 다른 종류의 유독 가스에 중독 될 수 있고
  • 환기 시설이 잘 되지 않은 장소에서 연탄을 땔 때 일산화탄소에 중독 될 수 있다.

산화탄소 중독의 증상 징후

  • 호흡하고 있는 공기 속에 들어 있는 일산화탄소 농도, 그런 농도의 일산화탄소를 얼마나 오랫동안 호흡했었는지,
  • 헤모글로빈에 붙어 있는 혈중 일산화탄소 농도의 정도 등에 따라 일산화탄소 중독의 증상 징후가 다르다. 
  • 헤모글로빈에 일산화탄소가 소량 붙어 있는 것은 정상적이다. 일산화탄소가 붙은 헤모글로빈을 COHb라 하고 그것의 정상 혈중 농도는 5% 이하 이다. 
  • 일산화탄소 혈중 농도가 20% 이상 될 때는 일산화탄소 중독의 증상 징후가 현저하게 나타나기 시작하고, 30% 일 때는 일산화탄소에 심하게 중독 될 수 있고, 60% 이상일 때는 사망하는 것이 보통이다( 질식 참조). 
  • 경도 일산화탄소 중독이 급성으로 생길 때는 머리가 무겁고 아플 수 있고 귓속에서 이상한 소리가 나고 집중이 잘 안 되며 어지러운 증상 징후, 근육통 등이 생길 수 있다. 
  • 일산화탄소에 심하게 중독될 때는 혼동하거나 구토하고 오줌똥을 싸고 전신경련(전신발작)을 일으키고 의식을 잃게 된다. 
  • 일산화탄소에 만성으로 경미하게 중독 될 때는 머리가 뻐근하게 아프면서 어지럽고 속이 메슥거리고 구역질이 나며 기운이 없고 현기증, 기억력 감소, 수면 장애, 신경염, 재발성 감염병, 복통, 적혈구 증가증, 신경정신 이상 등의 증상 징후가 생길 수 있다. 
  • 일산화탄소 농도가 정상 이상으로 높은 공기를 오랫동안 호흡하면 만성 일산화탄소 중독증에 걸릴 수 있다. 
  • 이때는 청력이 감소되고 보통 매일 하는 일의 능률이 오르지 않을 수 있다. 
  • 일산화탄소에 아주 심하게 중독 될 때는 눈동자의 크기가 정상 이상으로 커지고(산동이 생기고), 
  • 얼굴이 창백해지며 호흡을 빨리 하거나 호흡을 느리게 하고 의식을 잃고 전신경련(전신발작)을 하다가 결국 사망한다.

담배를 피우는 사람에게는 정상으로 본다.

일산화탄소 헤모글로빈 10% 일 때

육체적 운동할 때 숨이 조금 가쁘고 앞 머리가 조금 띵하고 얼굴에 홍조가 생기는 등 증상이 경미하게 나타난다.

일산화탄소 헤모글로빈 20% 일 때

육체적 운동을 하지 않는 상태에서도 숨이 가쁘고 앞 머리가 측두에 율동적 두통이 생기는 것이 보통이다.

일산화탄소 헤모글로빈 30% 일 때

신경이 예민해지고 두통, 피로, 판단 이상, 시야가 흐려지고 어지럼 등의 증상이 생긴다.

일산화탄소 헤모글로빈 40-50% 일 때

두통, 혼동, 육체적 운동시 실신, 기절 등의 증상이 생긴다.

일산화탄소 헤모글로빈 60-70% 일 때

의식 청명도가 상당이 감소된다. 경련, 호흡부전증이 생긴다.

일산화탄소 헤모글로빈 80% 일 때

짧은 시간 내에 사망한다.

  • 처음에는 무증상에서 시작해서→그 다음에는 피로와 두통, 불안과 우울증 등 증상으로 이어지고→신경정신 기능장애, 전신 피곤, 구기, 복통, 혼동, 현기증 등이 생기고→운동실조, 경련→심근경색, 뇌경색, 의식상실→사망으로 끝난다

일산화탄소 중독의 진단

  • 병력증상 징후와 진찰소견 등을 종합해 일산화탄소 중독을 의심되면 헤모글로빈에 붙은 일산화탄소 농도 즉일산화탄소 헤모글로빈(일산화탄소 혈색소/Carboxyhemoglobin/ CoHgb)의 혈중 농도를 측정해서 진단한다.
  • 연탄가스 등이 타는 밀폐된 장소에 있었던 사실엔진을 끄지 않은 채 놓아두었던 자동차 속이나 그런 자동차를 주차했던 차고 내에 오랫동안 있었던 사실 등을 참고로 해서 일산화탄소 중독을 진단한다(연기를 흡입했을 때일산화탄소 중독사고 예방 참조).

일산화탄소 중독의 치료

  • 일산화탄소 중독이 의심되면 현장에서 안전한 장소로 환아를 바로 옮기는 처치를 우선 할 일이다. 
  • 일산화탄소 중독의 정도에 따라 의료 구급대원이나 병원 응급실이나 단골 소아청소년 의사에게 긴급 전화 치료를 받고 그들의 지시에 따라 구급차로나 다른 적절한 방법으로 병원 응급실로 급히 데리고 간다.
  • 경미하게 중독 됐을 때는 신선한 공기를 호흡하게 하는 것이 치료의 전부이다. 
  • 그렇지만 심하게 중독 됐을 때는 헤모글로빈에 붙어 있는 일산화탄소를 헤모글로빈에서 빨리 분리시키는 치료를 하기 위해 기준기압 100% 산소 호흡 치료나 고압 100% 산소호흡 치료를 받는다. 
  • 이런 산소치료를 헤모글로빈에 붙은 일산화탄소(Carboxyhemoglobin/ COHb)의 농도가 5% 이하로 내려올 때 까지 치료한다. 
  • 중증 일산화탄소 중독으로 혼수상태에 빠져있을 때는 산소결핍증으로 뇌 손상이 이미 생겨 있을 수 있고, 뇌가 많이 부어 있을 수 있다. 이런 때는 이미 생긴 뇌부종을 적절히 치료해야 한다. 
  • 일산화탄소의 농도가 높은 공기를 같이 호흡했던 주위 사람들도 일산화탄소에 중독 될 수 있다. 때문에 같이 있었던 주위 사람들도 적절한 치료를 받아야 한다. 그리고 일산화탄소에 또다시 중독되지 않도록 예방한다. 
  • 일산화탄소의 농도가 정상 이상으로 높으면 경종을 울리는 일산화탄소 경보기를 집안에 설치해 일산화탄소 중독을 예방할 수 있다.

Overview of Carbon Monoxide Poisoning

• Breathing large amounts of high concentrations of carbon monoxide (CO) can cause carbon monoxide poisoning.

• Carbon monoxide poisoning caused by breathing high concentrations of carbon monoxide within a short period of time is called acute carbon monoxide poisoning.

• Carbon monoxide poisoning caused by breathing high concentrations of carbon monoxide for several days is called chronic carbon monoxide poisoning.

Carbon monoxide poisoning

• o Mild carbon monoxide poisoning;

o Moderate carbon monoxide poisoning; o Can be classified as moderate carbon monoxide poisoning.

• Hemoglobin is normally contained in red blood cells.

• The hemoglobin mainly plays a role in carrying oxygen into the alveoli during respiration to cells in organs and tissues of all systems of the body.

• When carbon monoxide and oxygen are present in the alveoli or blood, the affinity of carbon monoxide to hemoglobin is 200 times stronger than that of oxygen to hemoglobin.

• When you breathe more carbon monoxide than normal for some reason, hemoglobin carries more carbon monoxide to every cell in your body instead of carrying oxygen to cells in every organ and tissue in every system in your body. At this time, you can be poisoned with carbon monoxide.

• Depending on the severity of carbon monoxide poisoning, oxygen starvation occurs in all tissues of the body, and in severe cases, the cells of the tissues eventually fall into a suffocated state.

• In other words, when breathing air with a high concentration of carbon monoxide, instead of oxygen attaching to hemoglobin in red blood cells, carbon monoxide attaches to the hemoglobin and becomes carbon monoxide hemoglobin (Carboxyhemoglobin/Carboxyhemoglobin/COHb).

• As a result, hemoglobin cannot carry enough oxygen to the cells of the body’s tissues.

• In this case, the amount of hemoglobin in the body is normally sufficient, but oxygen starvation occurs in the body and eventually, carbon monoxide poisoning occurs.

• In other words, the blood hemoglobin level is normal, but symptoms of severe anemia may occur.

Causes of carbon monoxide poisoning

• Incorrectly installed heating system;

• Unsafely installed ventilation system;

• When burning petroleum, coal, or charcoal in an enclosed area with poor ventilation;

• When the vehicle engine has not been turned off for a long time in an enclosed vehicle;

• When you have been in an enclosed garage for a long time, in which the car engine has not been turned off for a long time;

• When breathing high concentrations of carbon monoxide from fire,

• In addition, you can get carbon monoxide poisoning when you breathe air with a high concentration of carbon monoxide.

• You can be poisoned by carbon monoxide or other toxic gases even if you do not get burned in a fire place.

• Carbon monoxide poisoning can occur when briquettes are burned in poorly ventilated areas.

Signs, symptoms of carbon monoxide poisoning

• the concentration of carbon monoxide in the air you breathe, how long you have been breathing that concentration of carbon monoxide;

• Symptoms and signs of carbon monoxide poisoning differ depending on the level of carbon monoxide in the blood attached to hemoglobin.

• It is normal for hemoglobin to have small amounts of carbon monoxide attached to it. The hemoglobin to which carbon monoxide is attached is called COHb and its normal blood concentration is less than 5%.

• Signs of carbon monoxide poisoning begin to appear conspicuously when the carbon monoxide blood level is above 20%, carbon monoxide poisoning can be severe when it is 30%, and death is common when it is above 60% (see suffocation).

• When mild carbon monoxide poisoning occurs acutely, the head may feel heavy and painful, strange sounds may be heard in the ear, difficulty concentrating, dizziness symptoms, and muscle pain may occur.

• Severe carbon monoxide poisoning can cause confusion, vomiting, urination, generalized convulsions (generalized seizures), and loss of consciousness.

• When chronic and mild poisoning with carbon monoxide, symptoms such as headache, nausea, nausea, low energy, dizziness, memory loss, sleep disturbance, neuritis, recurrent infectious disease, abdominal pain, erythrocytosis, neuropsychiatric disorders, etc. signs may occur.

• Breathing in air with an abnormally high concentration of carbon monoxide for a long time can cause chronic carbon monoxide poisoning.

• During this time, your hearing may decrease and you may not be able to perform your usual daily tasks.

• When carbon monoxide poisoning is very severe, the pupils become larger than normal (mydriasis),

• The face becomes pale, breathing rapidly or slowly, loss of consciousness, generalized convulsions (general seizures), and eventually death.

I see it as normal when driving at high speed

Carbon monoxide hemoglobin 15% or less

For those who smoke, it is considered normal.

When carbon monoxide is 10% hemoglobin

During physical exercise, symptoms such as shortness of breath, a slight tingling in the front of the head, and redness on the face appear mild.

When carbon monoxide hemoglobin is 20%

It is common to have shortness of breath and rhythmic headaches in the frontal part of the head even when not physically active.

When carbon monoxide hemoglobin is 30%

The nerves become sensitive, and symptoms such as headache, fatigue, abnormal judgment, blurred vision, and dizziness occur.

When carbon monoxide hemoglobin is 40-50%

Symptoms include headache, confusion, fainting during physical exertion, and fainting.

When carbon monoxide hemoglobin is 60-70%

Conscious clarity is significantly reduced. Convulsions and respiratory failure occur.

When carbon monoxide is 80% hemoglobin

dies within a short time.

• Signs of symptoms when the concentration of carbon monoxide increases and exposure to carbon monoxide increases over time.

• At first, it starts as asymptomatic → then it leads to symptoms such as fatigue, headache, anxiety and depression → neuropsychiatric dysfunction, general fatigue, vomiting, abdominal pain, confusion, dizziness, etc. occur → ataxia, convulsions → myocardial infarction, Cerebral infarction, loss of consciousness → death (Source; The New England Journal of Medicine, March 19, 2009, p.1218).

Diagnosis of carbon monoxide poisoning

• If carbon monoxide poisoning is suspected by combining the medical history, symptom signs and examination findings, the blood concentration of carbon monoxide attached to hemoglobin, i.e., carbon monoxide hemoglobin (Carboxyhemoglobin/CoHgb), is measured and diagnosed.

• Diagnose carbon monoxide poisoning by referring to the fact that you have been in an enclosed area where briquettes, charcoal, gas, etc. are burning, or that you have been in a car that has been left without the engine turned off or that you have been in the garage where you parked such a car for a long time (inhalation of smoke) When done, see prevention of carbon monoxide poisoning accidents).

Treatment of carbon monoxide poisoning

• If carbon monoxide poisoning is suspected, priority should be given to moving the child from the scene to a safe place.

• Depending on the severity of carbon monoxide poisoning, get emergency phone treatment from a medical paramedic, hospital emergency room, or regular pediatrician, and take them to the hospital emergency room by ambulance or another appropriate method as directed by them.

• For mild poisoning, breathing fresh air is the only treatment.

• However, in case of severe poisoning, in order to quickly separate the carbon monoxide attached to hemoglobin from hemoglobin, receive 100% oxygen breathing therapy at baseline pressure or 100% oxygen breathing therapy at high pressure. • Treat this oxygen treatment until the concentration of carbon monoxide (Carboxyhemoglobin/COHb) attached to hemoglobin drops to 5% or less.

• If you are in a coma due to severe carbon monoxide poisoning, you may have already suffered brain damage from anoxia and your brain may be very swollen. In this case, cerebral edema that has already occurred should be properly treated.

• People around you who breathe air with a high concentration of carbon monoxide can also become poisoned with carbon monoxide. Therefore, the people around you must also receive appropriate treatment. And prevent carbon monoxide poisoning again.

• Carbon monoxide poisoning can be prevented by installing a carbon monoxide alarm that sounds an alarm when the concentration of carbon monoxide is higher than normal.
