기관지 확장증, Bronchiectasis

 

기관지 확장증의 원인과 개요

  • 기관지의 상당한 부분이나 일부가 비정상적으로 확장된 것을 기관지확장이라고 한다. 
  • 그로 인해서 생기는 증상 징후를 기관지 확장증이라고 한다. 
  • 요즘 기관지 확장증은 보기 드문 병이다. 
  • 선천성 기관지 기형·
    • 기관지 내 이물·홍역이나 백일해 등 감염병을 앓을 때 생긴 후유증, 
    • 폐농양이나 폐종양 등에 의해 기관지 벽과 기관지 주위 조직이 손상될 때, 
    • 그 밖의 다른 원인으로 기관지 확장증이 생길 수 있다. 
  • 기관지의 일부나 상당 부위가 확장될 수 있다. 
  • 확장된 기관지 속에 분비된 점액이 섞인 가래나 고름이 섞인 가래가 괼 수 있고 그밖에 다른 여러 가지 증상 징후가 나타날 수 있다. 
  • 기관지의 일부가 선천성으로 확장되어 선천성 기관지 확장증이 생길 수 있고, 
  • 어떤 종류의 박테리아가 기관지와 폐에 감염되어 기관지염과 폐렴을 일으키고 그 병이 낫은  후 기관지의 일부, 또 대부분이 확장되어 후천적으로 기관지 확장증이 생길 수 있다. 
  • 폐낭종·면역체 결핍증, 부비동염, 폐기종, 기관지 천식을 비롯한 다른 여러 종류의 병들이 기관지 확장증을 일으킬 수 있다. 

기관지 확장증의 증상 징후

  • 원인·중증도·합병증의 유무 등에 따라 증상 징후가 다르다. 
  • 어떤 박테리아가 확장된 기관지에 감염 되면 급성 기관지염이 새로 생길 수 있다. 
  • 이때 노랗고 짙은 고름이 섞인 가래가 나오고 기침을 많이 하게 된다. 
  • 영유아들이나 학령기 아이들, 사춘기 아이들, 성인들의 일부는 기침할 때 기관지, 기관 등 하기도에서 입 안으로 나온 가래를 입 밖으로 잘 뱉어내지 못하고, 인두 강 속이나 입 안 속에 있는 가래는 삼켜 버릴 수있다. 
  • 가래에 피가 섞여 나올 수 있고, 기관지염이나 폐렴에 자주 걸릴 수 있다. 
  • 이런 정도의 기관지 확장증이 있을 때는 약물치료로만 잘 완치되지 않는다. 
  • 잘 먹지 않기 때문에 체중이 제대로 증가되지 않을 수 있다. 
  • 드물게 손가락 끝에 곤봉상지(사진 13 곤봉상지 참조)가 생길 수 있다. 
  • 이 병을 수술이나 다른 방법으로 근본적으로 치료하지 않는 한 일생 동안 이 병이 계속될 수 있다. 

기관지 확장증의 진단

  • 병력·증상 징후·진찰소견 등을 종합해서 이 병을 진단할 수 있다. 
  • 누구든지 기침을 장기간 할 때는 기관지 확장증으로 기침을 장기간 하나 적어도 한번 정도 생각해야 한다. 다행히 이 병은 그렇게 흔한 병이 아니다. 
  • 어떤 소아청소년이든 기침을 장기간 계속하다가 얼마 동안 안 하다가 또 장기간 기침할 때는 기관지 천식·기관지 내 이물·기관지 확장증 등이 있나 감별 진단해야 한다. 
  • 가슴 X-선 사진으로 진단하는 데 도움을 얻을 수 있다. 
  • 조영물을 기관지 속에 넣고 가슴 X-선 검사 사진으로 진단할 수 있다. 
  • 기관지 속을 들여다보는 기관지 내시경 검사로 진단할 수 있다. 
  • 원인에 따라 진단 방법이 다를 수 있다.

기관지 확장증의 치료

  • 기관지 확장증을 일으킨 근본 원인을 찾아 그 원인에 따라 치료한다. 
  • 백일해나 홍역 등을 앓다가 생긴 기관지 확장증을 치료하는 특별한 방법은 없다. 
  • 요즘 홍역 등으로 기관지 확장이 생겨 고생하는 아이가 흔치 않다. 
  • 일부의 기관지에 선천성으로 생긴 기관지 확장증은 확장된 기관지 부분을 수술 절제 치료해준다. 
  • 기관지 내 이물로 생긴 기관지 확장증은 기관지 내시경 이물 제거로 치료한다. 
  • 기관지 확장증이 있으면 근본 원인에 관계없이 환아의 머리와 상체를 하반신보다 15°정도 낮게 눕히고, 컵 모양으로 오므린 손바닥으로 환아의 앞가슴과 등을 가볍게 살살 두들겨서 기관지 속 가래를 중력에 의해서 입안 속으로 흘러나오게 체위 변경 물리치료를 한다. 심할 때는 한 번에 15~30번씩 하루에 4~5번 정도 이런 물리치료를 한다. 
  • 기관지 확장증이 있는 기관지에 박테리아 감염이 생기면 기관지염이 재발되어 기침을 더 심하게 하고 고름이 섞인 가래를 뱉을 수 있다.이때는 적절한 항생제로 7~14일간 치료한다. 
  • 이 병의 근본적인 원인을 진단해서 치료하고 기관지 확장증을 동시 치료한다.

Bronchiectasis

Causes and overview of bronchiectasis

• Abnormal enlargement of a significant part or part of the bronchi is called bronchiectasis. • The resulting symptoms are called bronchiectasis.

• Bronchiectasis is a rare disease these days.

• Congenital bronchial malformations•

• o Foreign bodies in the bronchial tubes, sequelae caused by infectious diseases such as measles or whooping cough;

o When the bronchial wall and surrounding tissues are damaged by a lung abscess or lung tumor,

o Other causes may cause bronchiectasis.

• Some or a significant portion of the bronchi may be dilated.

• Sputum mixed with mucus secreted into the dilated bronchi or sputum mixed with pus may be sputum, and various other

Symptoms, signs may appear.

• Congenital bronchiectasis may occur because a part of the bronchi is enlarged congenitally,

• Certain types of bacteria infect the bronchi and lungs, causing bronchitis and pneumonia, and after the disease is healed, part or most of the bronchi expand, resulting in acquired bronchiectasis.

• Pulmonary cyst

• Immune deficiency disease, sinusitis, emphysema, bronchial asthma and many other diseases can cause bronchiectasis.

• Symptoms and signs differ depending on the cause, severity, and presence or absence of complications.

• Acute bronchitis can develop when certain bacteria infect the dilated bronchi.

• At this time, sputum mixed with yellow and dark pus comes out and you cough a lot.

• Some infants, school-age children, adolescents, and adults may not be able to spit out phlegm from the lower respiratory tract, such as the bronchial tubes and trachea, out of the mouth when coughing, and may swallow the pharyngeal or pharyngeal sputum in the mouth.

• The sputum may be mixed with blood, and you may often get bronchitis or pneumonia.

• When there is this degree of bronchiectasis, it cannot be cured with medication alone.

• You may not gain enough weight because you do not eat well.

• Rarely, clubbing may develop on the tip of the finger (see photo 13 clubbing). • Unless the disease is radically treated with surgery or other means, it can last a lifetime.

Diagnosis of bronchiectasis

• This disease can be diagnosed by combining medical history, symptoms, signs, and examination findings.

• Anyone who coughs for a long period of time should consider at least one long-term cough with bronchiectasis. Fortunately, this disease is not so common.

• When any child or adolescent coughs continuously for a long time and does not cough for a long time, it should be differentially diagnosed for bronchial asthma, foreign body in the bronchus, or bronchiectasis.

• A chest X-ray can help with the diagnosis.

• Contrast can be placed in the bronchi and diagnosed by chest X-ray. • Diagnosis can be made by bronchoscopy, which looks into the bronchi.

• Diagnosis methods may differ depending on the cause.

Treatment of bronchiectasis

• Find the root cause of bronchiectasis and treat it according to the cause.

• There is no specific treatment for bronchiectasis caused by whooping cough or measles. • These days, it is rare for a child to suffer from bronchiectasis due to measles, etc.

• Bronchiectasis caused by congenital bronchiectasis in some bronchi is treated by surgical excision of the dilated bronchi.

• Bronchiectasis caused by a foreign body in the bronchi is treated with bronchoscopy.

• If there is bronchiectasis, regardless of the underlying cause, lay the child’s head and upper body 15° lower than the lower body, and gently tap the patient’s chest and back with the palms closed in a cup shape to release the phlegm from the bronchi into the mouth by gravity. Physiotherapy to change the position so that it flows out. In severe cases, this type of physical therapy is performed 15 to 30 times a day, 4 to 5 times a day. • Bronchiectasis with a bacterial infection in the bronchi can cause bronchitis to recur and make coughing worse and sputum mixed with pus. In this case, treat with appropriate antibiotics for 7 to 14 days.

• Diagnose and treat the underlying cause of this disease and treat bronchiectasis simultaneously.
